Filling the certificate
The assessee, who engaged you, and the acknowledgement downloaded in your presence.
Two steps of entry, then review.
Step 1 — Assessee & Engagement
- Select Existing Clients
Fills the assessee's particulars from a client record.
- Assesseerequired
Exactly one. Name of Assessee and Permanent Account Number (PAN). The PAN is printed in the Opinion as the login the acknowledgement was downloaded from, so an invalid one is refused.
- Engaged Byrequired
Paragraph 2 reads "I/we have been engaged by Mr. / Ms. / M/s …". Usually the assessee. Type the title you want printed.
- Submitted Torequired
The bank or authority the certificate is for. It is addressed to them, and named again in the scope and the restriction on use.
- Their Address
Printed under their name in the "To" block.
- Date of Engagement Letterrequired
Paragraph 1.
- Certificate Daterequired
Step 2 — The Acknowledgement
Everything here fills the Opinion.
- Assessment Yearrequired
One per certificate. It is the assessment year, not the financial year — income earned in
FY 2025-26is returned inAY 2026-27.- Acknowledgement No.required
As printed on the ITR-V. Digits only.
- Date of Filingrequired
As printed on the ITR-V beside the acknowledgement number. It appears on the ITR V page as the date of filing, and in the Opinion as the date the acknowledgement is dated.
- Date of Downloadrequired
The day the assessee downloaded it from their own login while you watched.
- Has the Return Been Verified?required
If not, the certificate cannot be issued — see About this certificate.
Warning
Copy the acknowledgement number and date from the document the assessee downloaded in front of you, not from a copy they brought. That download is the thing you are certifying.
Step 3 — Review & Issue
As with every certificate: the preview, Select Issuing CA Firm, and the UDIN Number.
